Preschool Requirements


 
REQUIREMENTS FOR PRESCHOOL 
                   
 
 
                    1. Immunizations

 All students, prior to entering Preschool, must have the 
              following immunizations & physical exam:

 
 
 

DPT/DtaP

Four doses

Polio    

Three doses

MMR

One dose of MMR (Measles/Mumps/Rubella) on or after your child's first birthday.

HIB

At least one dose on or after your child's first birthday.

Pneumococcal

Varicella  

At least one dose on or after your child's first birthday.

__________________________________________________________
 
 One dose on or after your child's first birthday.
 

Influenza/Flu 

 By December 31st of each school year (preschool only)

Physical Exam  

A current physical exam given by your child's physician is to be completed prior to entering Preschool. This should include both vision and hearing testing and be no older than 365 days before the start of Preschool.  If your child has a well visit scheduled in the month of September, please contact the school nurse and provide the appointment date.
 
 
Children without the required immunizations and/or physical examination will NOT be permitted to
remain in school beyond a 30 day grace period.  Proof of your child's immunizations must be
recorded on the papers given to you at kindergarten registration and stamped and signed by your
